Complete the Application and include all the required information below:

1. Application completed and signed by student.

2. Non-refundable registration fee ($55) MUST be submitted with application.

3. One letter of recommendation.

4. State background check

Click on 'Request A Search' and complete the information.

This must be completed BEFORE any clinicals can be started. We urge you to complete this before the first day of class.

5. State or Federal photo identification.

6. Social Security card.

7. Minimum 18 years old. (Must have proof of age: drivers license, birth certificate, State ID)

8. Proof of High School Diploma or GED.

9. Documentation of a physical exam performed by a MD or DO within the last 12 months.

10. Documentation of MMR or Antibody Titer Test, Current Hepatitis B Vaccination and Negative PPD in the last 12 months.
